2. Gol D. Roger – One Piece
Gol D. Roger is known as one of the most powerful characters in One Piece. He could match the strength of Whitebeard, who was once recognized as the strongest man in the world.
However, while Whitebeard lived to be 72 years old and Rayleigh lived to 78, Roger passed away at the age of 53 due to a terminal illness. Roger was afflicted with this illness during his final voyage to Laugh Tale. When Roger faced the Marines, the Pirate King’s lifespan was already limited.
3. Uchiha Itachi – Naruto

4. Juushiro Ukitake – Bleach
Juushiro Ukitake from Bleach is one of the characters with the highest potential for immense strength. Unfortunately, since childhood, Ukitake has been afflicted with an illness that causes him to cough up blood and has effects similar to a debilitating weakness.

5. Izumi Curtis – Fullmetal Alchemist
Izumi Curtis is one of the strongest alchemists in Fullmetal Alchemist. In reality, she possesses greater power than both of her students, Edward and Al. She has gone through intense training to survive, which has shaped her into a formidable alchemist.
However, Izumi’s health is a serious concern. She once attempted to perform human transmutation to bring back her deceased child but failed and suffered dire consequences. After that, Izumi frequently coughed up blood.
Her condition became notably severe in the first Fullmetal Alchemist anime, where she ultimately died before the events of Conqueror of Shamballa.
